- Application form for consular certification according to Model LS/HPH-2012/TK or print from website lanhsuvietnam.gov.vn.
- Original identification documents: Identity card, passport or documents that can replace passport (if submitted directly). If submitted by post, you need to provide 01 copy of identification documents.
- Documents and papers requested for consular certification: Provide originals or certified copies.
- 01 copy of documents and papers requested for consular certification.
- 01 envelope with the recipient's address clearly written (if the application is sent by post and the results are requested to be returned by post).
When needing consular certification of foreign documents and papers for use in Vietnam
- Consular certification/legalization application form number LS/HPH-2012/TK or online here
- Original identification documents: Identity card, passport or documents that can replace passport (if submitted directly). If submitted by post, you need to provide 01 copy of identification documents.
- Papers and documents requesting consular legalization: Papers and documents must be certified by a foreign diplomatic mission, consular office or other agency authorized to perform consular functions.

- 01 translation of documents and papers requesting consular legalization into Vietnamese or English (if necessary). Translations do not need to be certified.
- 01 copy of translated documents.
