Passport application (first time, re-issue, change):
a. First level:
- 01 declaration form according to the prescribed form (form TK/XC).
- 04 new photos, size 4×6 cm, face looking straight, bare head, white background.
* For children under 14 years old:
- The application form for a passport according to the prescribed form must be confirmed and stamped by the police of the commune, ward or town where the child is a permanent or temporary resident.
- In case of requesting to issue a passport together with the mother or father, submit 01 copy of birth certificate (bring the original for comparison) and 04 3x4cm photos.
- In case of requesting a separate passport, submit 01 application form according to the prescribed form (mother and father fill in and sign the application form); 01 copy of birth certificate and 4 photos size 4×6 cm.
- In case the mother or father is no longer alive, the adoptive mother, father or guardian (with documents proving that he or she is the legal adoptive mother or father) must declare and sign the declaration.
b. Reissue passport (passport is lost; damaged; valid for less than 30 days or expired; separating children from the mother's or father's passport), in addition to the provisions in section a above, additional documents must be provided in the following cases:
- In case of lost passport, submit a report of lost passport as prescribed in Point 3, Section II of this Circular.
- In case the passport is damaged or the passport is valid for less than 30 days, return the passport.
- In case of separating children from the mother or father's passport, submit the passport, 01 declaration form according to the prescribed form and 02 photos of the mother or father to reissue the passport; submit 01 declaration form according to the prescribed form and 02 photos of the child to issue a separate passport for that child (the declaration form must be filled out and signed by the mother or father).
c. Change passport (passport valid for at least 30 days; passport valid but out of visa pages or for other reasons), in addition to the documents specified in section a above, the passport must be submitted as a basis for renewal.
- The passport renewal application form does not require confirmation from the police of the commune, ward or town where you permanently or temporarily reside.
